Request to Add a Graduate Certificate

    This application form is for currently enrolled graduate students who have already been admitted to a degree program at The University of Toledo and who have paid graduate fees. If you have been absent for one calendar year or more AND you wish to complete the program to which you were previously admitted you are required to complete the re-admission process. You should not submit this form until you have been re-admitted.

    This form serves as the application to change your degree within your program (e.g. Master’s to PhD) or to change your graduate program of study. You are responsible for all application materials.

    If you have received a graduate degree in a given department/program you may not pursue the same degree again, even in a different area of specialization. For example: a student cannot receive two Masters of Arts in Education (any school) but can receive both a Master of Arts and a Master of Education.

    Please note that assistantships do not automatically transfer with you to your new graduate program. We recommend that you first contact your proposed graduate program to gain a clear understanding of their requirements.

    International students must also contact International Student Services regarding changes to their program of study.
    Processing Steps
    1. The student completes the Request to Add a Graduate Certificate form.
    2. The Graduate College reviews and forwards the request to the proposed graduate program for an official decision.
    3. Once a decision is made by the proposed graduate program, the Graduate College will process it and the student will receive an official decision letter.
